First of all, he was interested in records and rumors about relics that a monk could use. Two of them captured his attention: Skull of the Dragon and Mask of Cernunnos. While I agree with "+1 common -> +2 uncommon -> +3 rare" progression, there is a point for clarification. This is only my opinion as a player, but +2 should be available not only as loot. Let non-loot +2 have less bonuses than loot, let the price be very high (taking into account the players who save gold in the classical way and not by crafting, otherwise the price will be too high), let it be very hard to make. In some cases let it be a quasi-magical item. But give players a direct way to get it after predictable amount of time investment.
+2 does not reduce the value of divine and arcane buffs. +2 gives a chance to a fighter, while there is no one in the game to buff him. +2 is a good middle ground because it borders with lower and higher levels of enchantment. And if the problem is in the balance of monsters, and not balance of the items, then it may be worth starting the changes with the monsters? This will give a chance to introduce those monsters that should be in the setting, and to remove those who should not be here. Just my opinion as a player.  Sorry if my point of view is not clear, or the text is too muddled. English is not my forte.

In the forest near the southbank. A small crater covered with patterns resembling lightning. In the center is a charred skeleton with hands torn off. Both the crater and the skeleton are covered with metal and crystal shrapnel, which in the past was obviously small details.

Thelgrim came to the library in search of reports from a local observatory or astrologers regarding observations of falling meteorites. Among the reports he chooses those that describe small, high-density meteorites, showing just a small loss of visible mass in the atmosphere. Then in already selected report he searches for those with an area of impact in the wild lands to reduce the chance that an expedition was sent to claim meteorite.
The ultimate goal is to determine the meteorite with the highest chance of containing a small amount of Star Metal and calculate the approximate coordinates of its landing.

I see a lot of back and forth here but this is an issue that is being looked at from one angle and addressed by another. The DMs (seem, from what I can read) want death to be something to be feared ICly. Yet all offerings are mechanical, OOC repercussions. The consequences do not match the goal.

So that was spinning in my head all this time! Exp loss have no connection at all to RP consequences. Either this, or level is important for RP. Indeed, players face not something interesting from the character development perspective, but just more of the grind.
But finding a good solution for such a problem is even more difficult. Grendel have some interesting suggestions in this direction.

Sometimes it's month or two worth of exp. Of course loss of exp sucks regardless of your character coolness and involvement in current plots. It would suck less if 16+ character could restore previously achieved as effectively as 10+. Ultimately, the peak has already been reached by them. Yes, this is a role-playing game. But it is also a computer game. Both of these parts are important, both should bring fun, and neither diminishes the value of the other.
In addition, the DnD is a system in which you control the "super-humans" with an unhealthy amount of heroism. Is realism appropriate here?

Suggestion, though requiring a scripting, is to change this spell entirely into the actual summoning of actual tentacles. Like the upperdark ones but weakened, downscaled, a bit slowed and with the blackening vfx applied.

Something like, for example, caster level/3 tentacles for caster level/2 duration per cast.

Well... Maybe it's technically impossible, I do not know. But this awesome idea was suggested during discussion in the discord. Expertise stance should give the spell failure chance making it unattractive for the spellcasters.

I will be brief. Expertise has always been helpful to spellcasters and they used it, almost nothing in this sense has changed. Had to turn it on after using spells, but this is a minor limitation. The current implementation affects not only the expertise but also the power attack and makes the stances more useful for melee fighters. This was the goal of the change and the players themselves asked for it.

The correct question is maybe - is it possible to divide the actions into those that interrupt the stance and those that do not interrupt the stance?
